Disclosure & Incident Reporting Process & Forms
Have you experienced an incident during mentoring or need to report a child safety concern?
Child abuse is any action that harms a child. It may be physical, emotional, psychological, verbal or sexual. Abuse can happen anywhere and by anyone.
What to say when a Child Discloses
You may be the first person a child tells. Your role is to support, listen, make a report. If a child makes a disclosure, you should thank the child for sharing and gently let them know you will need to tell someone to help keep them safe. Then return them to their classroom or wellbeing staff and report to the Principal or School contact immediately.
Step 1 How to Report to the School
If an incident occurs (injury, concern, or disclosure), act immediately:
Report to the school first (immediately)
Inform the principal or nominated staff member
Follow the school’s reporting procedures. This may involve completing documentation or participating in a meeting or discussion as part of the school’s reporting process.
Step 2 Notify Kids Hope (online forms below)
After you have reported to the school and followed their process,
Notify Kids Hope (same day)
- Choose the correct form to submit: A Disclosure made by a child, or an Incident involving an adult mentor
Disclosure Form (Child safety/disclosure):
This form is used to report any suspicion, allegation, or disclosure of child abuse, or any concern relating to the wellbeing of a child.
Incident Report (For situations involving mentors)
This form is to be used to notify Kids Hope of any incident involving injury or harm to a Mentor, or damage to a Mentor’s personal property while on school premises. It is also used to report any concerns about the conduct or behaviour of a Kids Hope volunteer.
Keeping children safe is our highest priority.
If you have reported a disclosure or incident, you may also seek support from a coordinator, pastor, or health professional in a confidential setting.
